Ciclesonide is a synthetic corticosteroid typically used to treat inflammation, swelling and excess mucous production in the airways.

The medication is more commonly known by its brand names:

• Alvesco MDI is an inhaler used in long-term management of asthma, COPD, or certain other chronic breathing conditions.
• Omnaris (also known as Omniair) is a nasal spray used to prevent or manage seasonal and year-round nasal allergies, often referred to as hay fever.

The medication's action is similar in both uses; it reduces inflammation in the airways and inhibits the immune system's response to allergens. With regularly use, asthma attacks and hay fever symptoms should develop less frequently, and when symptoms do occur, they should not be as severe as when untreated.

Managing Asthma

Ciclesonide is very similar to any other inhaled steroid when used to manage asthma or other chronic breathing ailments. It is typically taken twice a day for as long as it remains effective. Note that it will have no effect on asthma symptoms during an attack; do not buy ciclesonide for use as a rescue inhaler.

As an inhaler, patients can buy ciclesonide in two strengths:

• 80 mcg per puff
• 160 mcg per puff

This is for convenience only; patients who need to take 160 mcg per dose may find it easier and more convenient to do so with one puff of a 160 mcg inhaler than 2 puffs with an 80 mcg inhaler, but it doesn't matter how the dose is delivered as far as effectiveness goes. • If after 4 weeks of treatment no significant improvements are noticed, increase each dose by 80 mcg for a total of 320 mcg.
• Do not exceed 320 mcg per day without explicit doctor approval.
• Rarely 320 mcg will be taken twice daily for a total of 640 mcg.
• Spread daily doses as evenly apart as possible, and try to take doses at the same time each day.
• Missed doses may be taken as soon as remembered.

Many patients notice small improvements after just a few days of treatment, but it can take up to a month for full effects to be felt.

Managing Nasal Allergies

Omnaris nasal spray comes in a metered-dose spray pump; each spray contains 50 mcg ciclesonide. It is not possible to buy ciclesonide in a higher dosage as a nasal spray.

The product may be used in several ways:

• To manage existing allergy symptoms;
• To prevent seasonal allergies;
• To prevent year-round allergies.

Dosage is the same in all cases:

• 2 sprays per nostril once daily, for a total of 200 mcg.

Length of treatment can vary depending on purpose:

• To manage existing allergies, start use at any time and continue until no longer exposed to allergens.
• To prevent seasonal allergies, start use 2 to 4 weeks prior to anticipated allergen exposure and continue throughout exposure.
• To prevent year-round allergies, use daily indefinitely.

Like inhaled ciclesonide, some patients may notice improvements in just days, but full effects will probably not be felt for 3 or 4 weeks.
